- Class B or higher California Commercial Driver’s License with passenger & airbrake endorsement or Class B Permit with endorsements
- H6 DMV printout –10 year driving history
- Continuous 10 years of employment history, employer addresses and phone numbers
- Proof of identification and proof of eligibility to work in USA
- Social Security Number
